As of December 15, 2022, Mason COVID Health Check will only be used for consent for COVID testing. If you plan to participate in COVID testing, please complete Mason COVID Health Check on the day of your test. Additionally, as of December 15, 2022, individuals are no longer required to report positive test results.
What is Mason COVID Health Check?
Mason COVID Health Check is a university application used for symptom screening and COVID test consent. This information is required to participate in Mason’s diagnostic and at-will COVID testing.
Based on your responses to the Mason COVID Health Check, you will receive an automated message.
|You have no symptoms associated with COVID-19 and you consent to testing; you can participate in at-will testing.|
|You have indicated that you may have one or more COVID-19 symptoms. Students should contact Student Health Services for diagnostic testing. Employees may select a testing appointment at the Peterson Hall diagnostic site.|
|You have declined consent to COVID testing. Please discuss your response with COVID testing staff if you are interested in receiving a COVID test.|
Consent to test is required for both at-will and diagnostic testing at our on campus sites.
All information captured by the Mason COVID Health Check is kept on a secure HIPAA compliant server.
